Telemedicine for Skilled Nursing Facilities
Virtual rounding, after-hours care, and specialist care technology for SNFs
SNF Virtual Rounding
Minimize COVID-19 exposure with telemedicine
Telemedicine is an indispensable tool during COVID-19 to help clinicians provide care to patients virtually. Telemedicine is particularly necessary in the skilled nursing setting, where elderly and vulnerable patients need to be isolated as much as possible from exposure. CureCompanion’s SNF telemedicine technology allows physicians to round virtually, without having to go into the facility. Not only does this mitigate the spread of COVID-19, it also reduces strain on the medical workforce.
Our advanced technology allows healthcare providers to perform virtual medical exams which are on par with in-person exams. Providers can not only see and talk to the patient, but also hear their heart and lung sounds, examine skin, eyes, ears, and throat, and get key vitals and diagnostics of the patient virtually.
Reduce Readmissions with Telemedicine
Enable virtual care with your own doctors
Hospital readmissions for patients at skilled nursing facilities (SNFs) are associated with adverse patient outcomes and reduced quality of life. Moreover, readmissions are very costly for SNFs because not only do they lose revenue when the patient leaves their facility, they are also penalized by CMS if their readmission rate is too high (penalties are temporarily suspended through 2022 due to COVID-19).
CureCompanion’s SNF telemedicine technology allows the facility’s existing on-call physicians to perform detailed medical exams virtually and make better judgments on whether the patient needs to be readmitted to the hospital.
Research has shown that consistent use of telemedicine improves readmission rates at SNFs. With COVID-19, it is more important than ever for SNFs to leverage the power of telemedicine to improve quality of care for their patients and reduce strain on their medical staff.
Enable Specialist Care, Virtually
Allow patients to access specialists without needing to travel
Specialist physicians generally do not visit skilled nursing facilities, so if a patient needs specialist care, they must be transported to and from the specialist. For SNF patients, this can be a cumbersome and stressful event, so most patients forgo specialist care while at the SNF.
With CureCompanion’s SNF telemedicine technology, patients can gain access to the specialist care they need virtually, without having to leave the facility.
How It Works
1. Secure Video Calling
The nurse calls the on-call provider using our software platform. The provider sees and talks to the patient using their own iOS/Android mobile device, laptop, or PC.
2. Virtual Examination
The provider examines the patient's skin, eyes, ears, and throat virtually using medical-grade HD cameras.
3. Real-time Auscultation
The provider listens to the patient's heart and lung sounds virtually in real-time.
4. Vitals and Diagnostics
The nurse records the patient's vitals wirelessly using our Bluetooth medical devices. The provider reviews them before/while talking to the patient. Vitals are automatically synced to PointClickCare.
5. Patient Records
The provider reviews symptoms and past medical history retrieved from PointClickCare or entered by the nurse.
